The case of a 60-year-old woman who experienced a 4-month history of reddish papules and plaques of the face and of the chest is reported. Mild itch was associated. Histology showed a lymphocytic dermal infiltrate, with exocytosis into the epidermis. Patch test was performed. A reaction to fragrance mix with a pattern similar to the skin lesions was registered. Histological pattern was similar, too. The patients healed after avoidance of fragrances and a few days of therapy with a topical corticosteroid. Lymphomatoid contact dermatitis (LCD) is an allergic contact dermatitis with clinical and histological findings mimicking a cutaneous T-cell lymphoma. LCD is now classified as a pseudolymphoma. Many substances can produce LCD; at the best of our knowledge, this is the first case due to fragrances
