La sanità di prossimità: un progetto di rigenerazione person centred

The aim of this work is to illustrate a proximity healthcare project implemented in an inner area of the Molise region: the Fortore area. The description of the proximity healthcare project aims to demonstrate that involving local communities in the governance processes of this function aims to rebuild the trust between the welfare system and the local community, placing the individual at the center. This is evidenced by the pursuit of guaranteeing essential levels of healthcare by activating citizens themselves and their networks to build more appropriate and effective forms of care and assistance. The proximity medicine described in this work is an approach to healthcare in which proactive efforts are made to engage with individuals to promote their health, prevent and diagnose diseases early, and provide care and socio-health services to citizens, the elderly, vulnerable individuals, and chronic patients through proactive facilities, services, and personnel located within and close to communities.
