The increasing lifespan causes a rapid growth of incidence and prevalence of STEMI in the elderly population. The management of the elderly STEMI patient is controversial according to the clinical complexity determined by clinical and angiographic characteristics. Furthermore, the absence or poor representation of elders in clinical trials is responsible of the lack of specific guidelines in this clinical subset. Although further evidence are still needed to improve the management of elderly patients with STEMI, encouraging results have already been obtained. Percutaneous revascularization has been shown to be superior in efficacy compared to fibrinolysis in elderly with STEMI with reduced frequency of the main and most feared risks as ischemia and bleeding.
