The increasing lifespan causes a rapid growth of incidence and prevalence ofSTEMI in the elderly population. The management of the elderly STEMI patientis controversial according to the clinical complexity determined by clinical andangiographic characteristics. Furthermore, the absence or poor representationof elders in clinical trials is responsible of the lack of specific guidelines in thisclinical subset. Although further evidence are still needed to improve the man-agement of elderly patients with STEMI, encouraging results have already beenobtained. Percutaneous revascularization has been shown to be superior in ef-ficacy compared to fibrinolysis in elderly with STEMI with reduced frequency ofthe main and most feared risks as ischemia and bleeding
