Most thyroid diseases are accompanied by alterations of the biosynthesis and/or secretion of thyroid hormones. Although iodine metabolism is still viewed as an essential part of thyroid hormone biosynthesis, emphasis has recently changed from iodine to the molecular site of hormonal synthesis, i.e., thyroglobulin (Tg). In this chapter thyroid hormone biosynthesis and secretion are viewed accordingly. Few areas of endocrinology have profited so much from advances in biochemistry and molecular biology as has clinical thyroidology. Whar follows is intended to be a critical appraisal of the subject from the viewpoint of the molecular endocrinologist.
